  • Define personal goals such as long-term housing, skills development, and community access.
  • Create a structured, measurable case plan with timelines and supports.

Progress Monitoring

Purpose: Track progress, celebrate wins, and adapt support as needed.

  • Regular case reviews and feedback sessions.
  • Adjust strategies to reflect changing needs or goals.
  • Maintain a trauma-informed and culturally sensitive approach that builds trust and motivation.

Transition to Long-Term Housing

Purpose: Support independence and stability beyond the program.

  • Identify suitable long-term housing options: including our Second Steps Forward Program
  • Build daily living skills and readiness for independent living.
  • Coordinate with housing providers and community services for a smooth transition.

Post-Exit Follow-Up

Purpose: Ensure lasting success and prevent relapse.

  • Maintain regular contact after exit for continued encouragement and guidance.
  • Offer check-ins, referrals, and problem-solving support.
  • Strengthen independence and community connection to prevent homelessness or re-incarceration.
